Essentials at a Glance

Program Type: Community-Based Services

To Make Referrals: Referrals come from Orange County Health Care Agency, long-term care facilities, and the community.

Who We Serve:

  • Adults, 18 and older, who are residents of Orange County.

  • Persons diagnosed with a serious mental illness who may have a co-occurring substance use disorder, and who may be privately or publicly LPS conserved.

Hours of Operation:

  • Monday - Friday: 8:00 a.m. to 5:00 p.m.

  • Evenings and Saturday: By appointment only

  • Sunday: On-call staff

  • Staff are available on-call 24 hours a day and can be reached at 714-245-6881

About the Program: At Telecare’s Striving Towards Enhanced Partnerships (STEPS) program in Orange County, you have the opportunity to connect with a group of people who are here to be your support team.

Our members come to us primarily from long-term care facilities and frequently are on LPS conservatorship. We believe that recovery starts from within and will do whatever it takes to support you on your recovery journey. We have a multidisciplinary team that includes psychiatrists and nurse practitioners, nurses, a master’s-level team lead, a clinical director, personal service coordinators with experience working in mental health and co-occurring substance use, and specialists in housing, education and employment, who are all here to help you on your path. STEPS staff are trained to address the special needs of our members on LPS conservatorship and their families. We have a clinician who provides support to members and their families or significant others. We work closely with residential rehabilitation and board and care providers, where most of the STEPS members reside, to ensure collaboration of services.

Your recovery journey is guided by an individualized service plan. You help create this plan by clarifying your needs and goals moving forward. We help you enhance your recovery process though activities that empower and assist you with stabilizing your health, and by partnering with community services to ensure continuity of care outside of the program.

Telecare is a great place that targets my mental and emotional instability, and stabilizes it by providing a support network that without it, I would be hopeless and lost.
Telecare is the best because they help me with my housing. I take classes here; they have helped me with getting a job and learning about my illness, and provide coping skills and teach me how to live on my own budget.
Telecare is such a nice program. Telecare has helped me a lot in my life. It has helped me to get a place to live. I like all the staff here, they have helped me get out of the hospital, and provide me comfort and a better lifestyle. Without Telecare, I would have been someone in bad shape, I would have been homeless or something. It’s a serious situation. I didn’t realize how serious it was until it was all gone. I appreciate them helping me to find a place to live. When I call the on-call, they always talk to me.
Telecare has helped me want to work and take care of myself and be able to succeed on the streets. I am happy to be out of the hospital. I have been in 10-15 hospitals. I talk to the staff and have conversations with them. I like going out with the groups, especially to the beach and paddling. I talk to the doctor about the things I don’t like about my medications. I feel happy and I feel safe when I am at Telecare. Me and my PSC took the bus to the mall. Before I was afraid, now I feel secure on the bus.
My life prior to coming to Telecare, I was very disrespectful, ornery, and rude to family members. Friends didn’t understand what was going on with me. I was using drugs almost every day, and I was on and off my meds. Then I ended up in jail due to using drugs and not taking medications as prescribed. When I got to Telecare, I got stable on my meds, I had a safe environment to come to, engaged in activities and groups, and got to meet people like me. It seemed like everyone was similar and the staff were great. Being here has allowed me to take care of my responsibilities and mend the relationship with my parents. I have also dropped friends that were a bad influences and that were on a bad track, and worked on the relationships with friends that wanted to be a part of my journey. I have been able to make changes for myself. I have gained new friends at Telecare because they also have similar goals and interests like my own. We attend groups and see each other at Telecare.
