Program Type: Permanent Supportive Housing (PSH) & Transitional Housing

To Make Referrals: All referrals to Canyon Vista Center PSH come from San Mateo County Behavioral Health and Recovery Services (BHRS).

# of Units: 57

Who We Serve: San Mateo County residents who have a serious mental health diagnosis and may have co-occurring substance use disorders are eligible for either Transitional Housing or Permanent Supportive​ Housing as determined by BHRS. The individuals may have been homeless or are at risk of homelessness due to other circumstances. Transitional Housing tenants are tenants who are working towards more independent living and may reside in Transitional Housing for up to 24 months. The PSH tenants may be previous tenants in Transitional Housing from this campus or another part of BHRS’s network of care and who are interested in living at Canyon Vista long-term.

Hours of Operation: 7 days a week, from 8:00 am - 7:00 pm.

About the Program: In partnership with County BHRS and property management, Telecare offers onsite supportive and case management services for Canyon Vista Center PSH residents. 

Service participation is voluntary and not required to reside at Canyon Vista Center. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: onsite in the community space, in your unit, on the campus, or in the larger community, if appropriate. These services focus on housing stability and retention, including but not limited to: 

  • Outreach and engagement services addressing housing stabilization and case management needs. 

  • Behavioral health assessment, screening, care coordination, and referrals for residents at high risk and needing more intensive behavioral health services to maintain housing. 

  • Addressing ongoing behavioral health care coordination needs. 

  • We offer site-wide community integration opportunities (BBQs, celebrations, tenant education, and other groups).
