Essentials At A Glance

Program Type: Community-based

To Make Referrals: The Vida ACT program has a closed referral process. All candidates must be referred by the Office of the Public Defender’s Defense Transition Unit (DTU) and the San Diego Sheriff’s Department. Referring parties must use the County of San Diego Single Point of Access System and submit referrals to SD_ReferralCenter@optum.com. All referrals must be screened and accepted before exiting custody and coordinated release to Vida ACT.

# of Members: 110 members

Who We Serve: Adults who have a serious mental illness, have a possible co-occurring substance use disorder, who are justice-involved, are eligible for probation, are on parole, and who may be homeless or at risk of being homeless.

Hours of Operation: Monday through Friday: 8:00 AM to 4:30 PM

About the Program: We believe that recovery starts from within and will do whatever it takes to support you on your recovery journey. We have a multidisciplinary team that includes a nurse practitioner, a program administrator, a licensed team lead, a peer recovery coach, mental health rehabilitation specialists, and case managers with specialties in housing, education, employment, and substance use who are all here to help you on your path.

Your recovery journey is guided by an individualized service plan. Our staff offers advocacy for individuals who have extensive contacts with the criminal justice system. We will assist you in recovery by tailoring services so you can better manage your conditions, monitor your progress toward your goals, adjust services to fit your changing needs, and partner with community services to ensure continuity of care.
