About CARE Court Outreach
Telecare is partnering with San Diego County Behavioral Health to open a new program dedicated to helping individuals with untreated mental illness engage and connect to services. CARE Court Outreach is part of California’s Community, Assistance, Recovery, and Empowerment (CARE) Act, a new approach to help unhoused individuals with severe mental illness engage with and access the support they deserve.
CARE Court Outreach is a person and client-centered, mission-driven program that reaches out to folks who might otherwise be unserved. Our goal is to help support people and fill the gap in our healthcare systems to create a meaningful entry into care.

Essentials At A Glance
Program Type: Community-based
To Make Referrals: A referral to CARE Court can be initiated by family members, behavioral health providers, first responders, or other approved petitioners, by filing a petition with the Superior Court. Referrals to Telecare CARE Outreach are made by the County of San Diego.

Residents of San Diego County, ages 18 and older.
Persons diagnosed with Psychotic Disorders
Hours of Operation: Monday to Friday: 8:00 am - 4:30 pm.
