At Telecare’s Alameda Assisted Outpatient Treatment (AOT) and Community Conservatorship (CC) program in downtown Oakland, our goal is to work closely with existing community resources to assist individuals with transitioning back into their community. We know that the journey to recovery can be a tough one, and we believe that no one should have to go through it alone.

Program Type: Community-Based

To Make Referrals: Call Alameda County ACCESS at 1-800-491-9099 for an AOT Screening. Individuals are evaluated by the county and referred to the appropriate level of treatment. AOT services are court-ordered.

Who We Serve:

  • Adults age 18 or older

  • Residents of Alameda County

  • Individuals who receive or are eligible for MediCal in Alameda County

  • Previous attempts to engage the individual in services have not been successful, but they have a demonstrated need for services

  • Individuals who are court-appointed to the program or assigned to a community conservator

Hours of Operation:

  • Monday - Friday: 8:30 a.m. to 5:00 p.m.

  • On-call staff availability 24 hours a day, 7 days a week

About the Program: Based on a recovery-centered model, AOT/CC of Alameda County is an intensive community support service and an Assertive Community Treatment (ACT) for individuals with severe mental illness (SMI), many of whom would otherwise require extended care in institutional settings. AOT/CC serves individuals who are high utilizers of mental health services and who are considered to be at great risk for psychiatric hospitalization. Our multidisciplinary team includes a psychiatrist, a nurse, masters-level clinical staff, and personal service coordinators who are all here to help individuals in our program on their path.

AOT/CC staff will walk with individuals on their journey in healing and provide a full range of services, including medical and psychiatric services, case management services, advocacy and linkage, referral to safe and affordable housing, substance use interventions and counseling, assistance with entitlements, support and education with family and significant others, connection with community resources, and self-help groups.
