New Long-Term Care Step-Down Program in San Diego: La Luz

Welcome, La Luz!

We are pleased to announce that La Luz, our new long-term care step-down program in San Diego, opened October 14, 2019! This program was opened in partnership with the San Diego County Department of Behavioral Health Services.

The La Luz program has been designed to help individuals successfully transition into their community; promote hope, wellness, and recovery; and to prevent unnecessary readmissions to long-term care hospitals. The goal of La Luz is to work with individuals to address current barriers so they can transition back into and achieve their goals in their community.

Address: The La Luz program is temporarily located until a permanent site is secured and renovated.

Who is Served: Adults who are homeless with a serious mental illness who may have a co-occurring substance use disorder, are not connected to outpatient services and are in long-term care facilities (LTC), and will be discharged as a step-down in to the community.

Hours of Operation: Monday-Friday 8:00 a.m. to 5:00 p.m.
