About the Program: At House of Transition, we serve individuals with serious and persistent mental illness who have been faced with multiple acute psychiatric hospital admissions and have difficulty maintaining a residence and their independence in the community.

We use various methods to connect with and support our residents in gaining skills and reintegrating into the community. These include regularly scheduled group sessions, appointments with psychiatric and medical staff, planned outings on campus, presentations about community resources, and involving families. We also take the time to recognize our residents' achievements throughout the month. Our members nominate and vote for a "Resident of the Month" who has demonstrated our clinical approach (RCCS) and surpassed their personal goals in social engagement. The chosen resident is acknowledged during group sessions, with their name and picture displayed on the bulletin board, and they are treated to a lunch of their choice.

Brighter Tomorrows and the Casa staff are here to support residents on their journey of recovery along with Ventura County Behavioral Health Clinical Teams. We believe “teamwork makes the dream work!”
